2017-2018 Midget FALL Tryout Registration Form

Barrie AAA Zone will begin their IN ZONE Midget age player (00-01's) tryouts on August 21st, 2017. OUT OF ZONE players may not attend until after August 23rd, 2017

Dates and times are as follows:

TRYOUT - Aug 21st - 5:30 PM @ Allandale Rec Center

HOME - Aug 25 10:00 AM vs. Central Ontario Wolves @ BMC

HOME – Aug 25 1:00 PM vs Peterborough @ BMC

HOME – Aug 25 4:00 PM vs. Peterborough Petes @ BMC

AWAY – Aug 26 10:00 AM vs Peterborough @ Kinsmen Arena

AWAY – Aug 26 1:00 PM vs Markham @ Kinsmen Arena

AWAY – Aug 27 2:00 PM vs Hamilton @ Gateway Arena

TRYOUT - Aug 28th - 10:00 AM @ East Bayfield

TRYOUT - Aug 28th - 2:30 PM @ East Bayfield

The registration fee is $125.00. 

  *NO EXCEPTIONS* CASH AND OR REGISTRATION IS NOT ACCEPTED AT THE ARENAS. 

Registration fees are non-refundable after the first tryout has begun. If payment was made prior to the first tryout and a refund is requested, a refund will be issued, less a $5.00 administration fee.

ALL midget players must provide an ORIGINAL signed PTS form from their home center.  Players whos home center is BMHA will not be required to bring the original PTS.  You MUST still request the PTS, but BMHA will notify Barrie AAA of your processed PTS.  

It is recommended you submit your PTS form request as soon as possible.

Players will not be permitted to tryout without a PTS form processed. NO EXCEPTIONS

Parent/Guardian Information

Registration/payment is NOT accepted at the arenas-no exceptions.
Players are reminded to wear proper equipment as per OMHA rules. This includes, mouth guards, neck protection and goalie 'dangles' (plastic neck guards). Players not wearing appropriate protection in compliance with OMHA rules will not be permitted on the ice surface.
Waiver of Liability

We, the parent(s)/guardian(s) of the above named candidate trying out for a position on a Barrie AAA Zone team, hereby give my/our approval to participate in any and all Barrie AAA Zone activities. I/we assume all risks and hazards incidental to such participation including transportation to and from the activities, and I/we do hereby waive, release, absolve, indemnify and agree to hold harmless the Barrie AAA Zone Hockey Association, the organizers, sponsors, supervisors, participants, and persons transporting my/our child, whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident or liability insurance.

Barrie AAA Zone have enabled an online payment system through PayPal. You can pay via a personal PayPal account if you have one, or via major credit card as a guest on PayPal. Debit cards are not accepted through PayPal. You will see that it says you can pay with a debit card, but this is not the same as our bank debit cards. When you have completed this form by answering all questions listed above, click Submit Form button shown below. Your registration information will be saved in our database. You will be taken to a new page, look to the bottom right and click 'Click Here to Pay Online' and you will be transferred to PayPal to complete the registration process by entering your payment information. To complete the registration process, you must complete this payment process.

Any registration forms submitted without the accompanying payment verification are not valid and your player will not be allowed to tryout. On the PayPal site, be sure to enter ALL required information, and when your payment information has been accepted. PayPal will email you a receipt of payment. Barrie AAA Zone will also be notified that your payment was successful.

Please print out your PayPal receipt and bring it with you to the rink for your players first tryout.
